STG SELECT Team Participation Agreement

Salute The Game • October 13, 2026 • Print, sign, and return to Coach B

1

Accepted-player enrollment

Team registration is for athletes accepted by Coach B or designated STG SELECT staff. Paying for a tryout does not guarantee a roster position. Before enrollment, the family must be told the selected team, season, participation dates, fees, and payment schedule and complete the required registration and approved consent documents. Tryout registration and team enrollment are separate.

2

Team dues and seasonal pricing

The signup fee is $500 for Spring/Summer and $375 for Fall/Winter, including late signups. Spring/Summer monthly dues are $285 during January–July. Fall/Winter monthly dues are $250 during August–December. Monthly dues are charged on the 5th. For a late signup, the first monthly dues payment is on the 5th of the following month. A signup fee applies to new players and at the start of a new travel year in January; continuing players do not pay another signup fee when moving into Fall/Winter. Registration must show the applicable signup fee and the first and last monthly charge dates before a family pays.

3

Recurring-payment authorization

The parent or guardian must separately authorize the disclosed recurring charges through checkout. Charges apply only to the selected enrollment and disclosed dates; a team change, new season, new signup fee, or changed price must be disclosed before payment authorization is obtained. Reading or acknowledging this page alone does not authorize a card charge. The approved policy and actual checkout and billing settings must match before enrollment opens.

4

Cancellation, departure, and refunds

Paid team fees are non-refundable except where applicable law requires otherwise. Cancellation concerns future charges and does not, by itself, refund earlier payments. A family may request cancellation by contacting Salute The Game. When a player leaves or is dismissed, the program must confirm the effective end date and stop future recurring charges after that date. Removing a player from a roster must also be accompanied by the required billing cancellation; a roster change alone is not confirmation that billing stopped.

5

Athlete and family expectations

Athletes and families are expected to attend scheduled activities as agreed, communicate absences and injuries, follow coach and safety instructions, respect teammates, officials, opponents, and facilities, and avoid bullying, harassment, or unsafe conduct. The program may remove an athlete from an unsafe activity and address conduct concerns with the parent or guardian. Team membership does not guarantee playing time, a particular position, or a specific competitive outcome.

6

Registration and parent acknowledgment

Required registration information requested by Coach B includes parent or guardian details, parent email, two emergency contacts, athlete first and last name, birthdate, age, grade, position, years playing competitive basketball, relevant medical issues, and signatures on the approved documents. Medical and contact details must be submitted through the private registration process. The agreement must identify the athlete, team, season, exact charge dates, parent or guardian, document version, and signature date.

Acknowledgment: I understand the selected team’s enrollment requirements, disclosed payment schedule, cancellation and refund terms, and conduct expectations. I will sign the agreement before team enrollment is completed.
